Our Policies

Thank you for choosing Ouachita Medical Clinic.  We realize that you have a choice in medical providers and are pleased that you have chosen to seek care with us.  The staff at Ouachita Medical Clinic strive to exceed expectations in care and service to make your experience with us as comfortable and stress-free as possible. Our goal is to provide quality medical care in a timely manner.  To do so, we have implemented the following office policies:

Walk In Availability

  • We accept walk ins daily for sick visits.

Appointment Scheduling

  • We offer online scheduling for the providers in our Monroe location through our AdvancedMD patient portal.
  • Motor vehicle accidents are seen on a case-by-case basis. Please call the office and speak to a receptionist to schedule these appointments.
  • Our providers do not accept any worker’s compensation cases.

No Show/Late Cancellation

  • Failure to show for a scheduled appointment without notice is considered a No Show and will result in the following fees: $25 for the first no show, and $50 for every subsequent no show
  • These fees will have to be paid in full before scheduling another appointment.
  • Please note that these fees are patient responsibility and will not be billed to your insurance.
  • Multiple no shows can result in dismissal from the practice at the provider’s discretion.
  • If you are unable to make it to your scheduled appointment, we ask that you give 24-hour notice. Failure to cancel within 8 hours of your appointment could result in a No-Show fee, unless emergency documentation is provided.

Medication

  • We will not refill controlled substances without an appointment.
  • We are not accepting any new chronic pain management or patients that take daily benzodiazepines (Klonopin, Xanax)
  • Please review your medications prior to your appointment and request refills at that time. Otherwise, refill requests could take up to 1-2 business days to complete.
  • We do not accept fax refill requests from your pharmacy. If a refill is needed prior to your appointment, please send a message through our patient portal.

Forms/Letters

  • We are happy to complete forms and write medical letters upon your request. However, there will be a fee charged for this service based on the complexity and time utilized for the completion of the requested documents.
  • The minimum fee for this service is $25.

Insurance/Payments

  • It is patient responsibility to inform our office of any changes in insurance coverage. Failure to do so could cause delay or denial of insurance payment.
  • Copay and deductible payments are due in full at time of service.
  • Self-pay patients must make payment in full for all services provided during your visit.
  • We accept cash, personal checks, Mastercard, VISA, American Express, and Discover. Checks should be made out to Ouachita Medical Clinic.
  • Past due balances must be paid in full prior to scheduling your next appointment.
  • If you have questions regarding your bill, please contact our billing department at 318-460-7880.

Demographic information

  • Patient must inform the receptionist of any change to their address or phone number. Changes can also be updated through the Advanced MD patient portal.

Patient Portal

  • In order to streamline patient correspondence, we ask that every patient sign up for our AdvancedMD patient portal.
  • Patients will be able to complete patient paperwork, send messages to our staff, schedule or cancel appointments, view results, and upload copies of their insurance cards or other documents that they would like to include in their chart.
  • Bill payments can be made through our portal with a credit/debit card.

Text Messaging Disclaimer

By providing your phone number to Ouachita Medical Clinic, LLC you agree and acknowledge that Ouachita Medical Clinic, LLC may send text messages to my wireless phone number for any purpose. Message and data rates may apply. Message frequency will vary, and you will be able to Opt-out by replying “STOP”.

Privacy Policy

No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
